Can Mounjaro or Zepbound Cause Hair Loss?

Mounjaro and Zepbound, both based on tirzepatide, have gained attention as injectable medications used for diabetes and weight management. As these treatments become more common, some patients are asking an important question:

“Can tirzepatide make my hair fall out?”

The answer is not simply yes or no. Hair shedding has been reported in some patients using tirzepatide, especially when weight loss happens rapidly. However, current evidence suggests that this hair loss is more likely related to rapid body weight reduction, nutritional changes, and temporary stress on the hair cycle, rather than direct damage to the hair follicles.

Why Hair Loss May Happen During Weight Loss Treatment

Hair growth is sensitive to changes in the body. When a person loses weight quickly, the body may interpret this as a form of physical stress. In response, more hairs may enter the resting phase of the hair cycle, which can lead to increased shedding a few months later.

This condition is known as telogen effluvium. It is a temporary type of hair shedding that can occur after rapid weight loss, illness, surgery, childbirth, severe stress, or nutritional deficiency.

In patients using medications such as Mounjaro or Zepbound, possible contributing factors include:

  • Rapid weight loss
  • Reduced calorie intake
  • Lower protein intake
  • Iron, zinc, vitamin D, or other nutritional deficiencies
  • Hormonal and metabolic changes during weight reduction
  • Existing androgenetic alopecia becoming more noticeable

This means that the medication may not be the only cause. In many cases, hair shedding occurs because the body is adjusting to a sudden change in weight and nutrition.

Is Hair Loss Listed as a Side Effect of Tirzepatide?

Hair loss has been reported in patients using tirzepatide-based weight loss treatment. In Zepbound’s prescribing information, alopecia was reported more frequently in patients receiving treatment than in those receiving placebo, and it was reported more often in female patients than male patients.

For Mounjaro, alopecia has also been listed among skin and subcutaneous tissue adverse reactions in prescribing information, although clear large-scale clinical frequency data remain limited.

Recent pharmacovigilance and observational studies have also suggested a possible association between GLP-1 receptor agonists, including semaglutide and tirzepatide, and hair loss reports. However, these findings do not prove a direct causal relationship. They show a signal that needs further research, especially because rapid weight loss itself is already a well-known trigger for telogen effluvium.

Mounjaro, Zepbound, Wegovy, and Saxenda: Is the Risk Different?

Among weight loss medications, hair loss reports appear to vary by drug and treatment context.

Zepbound, a tirzepatide-based medication for chronic weight management, has official labeling that mentions hair loss in clinical trial data. Mounjaro, also tirzepatide-based, is primarily used for type 2 diabetes, and alopecia has been listed as an adverse reaction. Wegovy and Saxenda, which are GLP-1 receptor agonists, have also been associated with hair loss reports, but the exact frequency and mechanism may differ depending on the medication, dose, weight loss speed, and patient condition.

In general, the stronger and faster the weight loss, the more likely the body may experience temporary stress. This is why hair shedding may be more noticeable in patients who lose weight rapidly within a short period.

When Does Hair Shedding Usually Start?

Hair shedding from telogen effluvium does not usually begin immediately. It often appears 2 to 4 months after a major physical stressor, such as rapid weight loss or nutritional deficiency.

Patients may notice more hair falling out during shampooing, brushing, or drying. In many cases, the scalp does not become permanently damaged. Instead, the hair cycle is temporarily shifted, and shedding gradually improves once the body stabilizes.

For many patients, hair shedding improves over several months when weight loss slows down, nutrition is corrected, and underlying hair loss conditions are managed properly.

How Can Hair Loss Be Managed During Mounjaro or Zepbound Treatment?

If hair shedding occurs while using tirzepatide, stopping the medication immediately is not always necessary. The most important step is to identify the cause and manage it properly with a medical professional.

Helpful management steps may include:

1. Avoid Losing Weight Too Quickly

A gradual pace of weight loss may reduce physical stress on the body. If weight is dropping too rapidly, the treatment plan, dose, diet, or exercise routine may need to be adjusted under medical supervision.

2. Check Nutritional Intake

Hair requires enough protein, iron, zinc, vitamin D, and other nutrients to maintain a healthy growth cycle. Patients who eat much less due to appetite suppression may unintentionally develop nutritional deficiencies.

3. Consider Blood Tests

Blood tests can help check for ferritin, thyroid function, vitamin D, zinc, anemia, and other factors that may contribute to hair shedding.

4. Treat Existing Hair Loss Conditions

If a patient already has androgenetic alopecia, rapid weight loss may make thinning more noticeable. In such cases, medical hair loss treatments such as minoxidil, finasteride, dutasteride, or other options may be considered depending on the patient’s condition.

5. Do Not Ignore Persistent or Severe Hair Loss

If shedding is severe, continues for more than several months, or is accompanied by scalp symptoms, a proper hair and scalp evaluation is recommended.

Is Tirzepatide Hair Loss Permanent?

In many cases, hair shedding related to rapid weight loss is temporary. Telogen effluvium usually improves once the body adapts, nutrition is restored, and weight stabilizes.

However, if a patient also has genetic hair loss, thyroid disease, anemia, severe nutritional deficiency, or another medical condition, the hair loss may not fully recover without treatment. This is why accurate diagnosis is important.

Conclusion: Mounjaro and Zepbound May Be Linked to Hair Shedding, But the Cause Is Often Indirect

Mounjaro and Zepbound do not appear to “destroy” hair follicles directly. Instead, hair shedding during treatment is more commonly understood as a temporary reaction to rapid weight loss, reduced nutrition, and metabolic stress.

For most patients, the key is not panic, but proper management. If hair shedding occurs, it is important to check the speed of weight loss, nutritional status, blood test results, and any underlying hair loss condition.

With the right approach, many cases of weight loss-related hair shedding can improve over time.
